Report an injury
Submit an injury report, add evidence, and correct your own record.
This entire incident, injury and investigation module is Complete-only. Standard hazard/JHA forms do not grant access to injury cases. Available actions also depend on role, site scope and rollout; see plans and billing.
Use the injury-report form to record what happened at the jobsite. Follow your site's emergency procedure first when someone needs immediate help; submitting a form does not contact emergency services.
Submit the report
Choose the site and time, describe what happened, and identify who was hurt: you, another person you can name, or someone not yet known. Add witnesses and photo or PDF evidence when available. Review the details before submitting.
Keep the description factual. Distinguish what you saw from what someone told you, and leave unknown details unknown.
Open or amend your report
You can open reports you submitted or are involved in, subject to the record's access rules. On mobile, open the injury report from Reports.
If you need to correct or add information, use the amendment action when offered and give a reason. Amendments are added to the history; they do not overwrite the original submission.
Medical details
The injured person can record their own medical details separately. Those details are restricted to the injured person and authorized incident managers. Do not put medical information into a general message, attachment, or narrative to bypass a missing medical-details action.
The report may show Recordability review pending. This is a review status, not a completed classification. Ask your incident manager about investigation or follow-up decisions.
